Our Team


Our dedicated team plays an integral part in the success of our patients at Lafayette Nursing and Rehabilitation. The members of our skilled and diverse therapy team have decades of combined experience with specialties in orthopedics, spinal treatment, neurological treatment, wounds, geriatrics, wheelchair positioning, restraint reduction, swallowing, cognitive staging and urinary incontinence. Our staff is compassionate and understanding to our patients' needs and foster a warm and positive atmosphere while utilizing their years of experience and skill to provide treatment.

Shayne Baumgardner
Executive Director, LNHA

Shayne is currently our Executive Director. Her career in health care began as a Certified Nursing Assistant in a long-term care facility at the age of 18 in 2008. Shayne knew right away that long-term care was her calling and that someday she wanted to become a Licensed Nursing Home Administrator. Over the years Shayne pursued her education while also maintaining a fulltime position. Shayne has worked in many different departments in long-term care such as the Receptionist, Business Office Manager, Admissions Coordinator, Admissions and Marketing Director and now Executive Director. Shayne graduated from North Florida Community College in 2015 with her Associates Degree. She continued furthering her education at Saint Leo University where she earned her Bachelors Degree in Health Care Management in 2018. She then successfully completed her six months of preceptorship and accepted the role of Executive Director in 2019. Shayne has a passion for the geriatric community as well as community service and it shows through her personality and spirit. Her mission as the Executive Director is to ensure exceptional quality of care for all of the residents’ and provide peace of mind for family members. At Lafayette Nursing and Rehabilitation we strive to maintain an atmosphere where the resident’s and their families feel at home.

Kayla Smith
Social Services Director and Admissions

Kayla is the Director of Social Services and Admissions facilitator at Lafayette Nursing and Rehabilitation Center. She began her journey into the healthcare industry in 2015 after graduating from Saint Leo University where she earned her bachelor’s degree in Psychology. She is currently continuing her education with Walden University where she will graduate with her master’s degree in Social Work. She is also a certified dementia practitioner and a certified advanced care planning facilitator. Kayla is passionate about serving the geriatric population and aspires to enrich the lives of each resident. She enjoys enhancing her knowledge and skills so that she can provide each resident with a variety of services individualized to meet their unique needs. Whether she is providing psychosocial support for resident’s transitioning into long term care or coordinating community-based services for those who are discharging home she aims to ensure the entire process is stress-free for the residents as well as their family members.
